Affinity is currently In-Network with:

• CIGNA

• Anthem Blue Cross Blue Shield (PPO plans only)

• CareFirst, including Federal plans (PPO plans only)

  Please note:

  *We are NOT in network with HMO plans, Anthem Healthkeepers, Medicare or Medicaid plans.

  *Please contact your insurance company or review their website to see if Christiane Davenport, NP or Bethany Morrell, NP are In-Network Mental Health Providers.

  * If our in-network plans (Cigna, Anthem, or CareFirst) are not your PRIMARY insurance, you will need to be Self Pay (pay upfront) and self submit for reimbursement.

FEES FOR TREATMENT:

Affinity also accepts Out-of-Network (self-pay) clients. You will be provided with an invoice (called a “superbill”) that you may submit to your insurance company for reimbursement.

You may check with your insurance to see what they will reimburse you for “out of network” providers for the following CPT codes:

• Initial Full Psychiatric evaluation - (CPT codes 99204/90836 or 99205): $350

• Follow-up Visits (CPT codes 99214 & 90833/90836): $195 - $230

CANCELLATION /NO SHOW FEES: 

A minimum of 24 hours notice is required to cancel a scheduled appointment. Otherwise, a No-Show fee of $100 will be charged.
